Support for Community Palliative Care Teams

In 2022, we launched 10 new ‘Community Palliative Care Teams’ to expand our community nursing model and help us care for significantly more patients each year.

Each of our 10 new teams is dedicated to caring for patients at home in a specific locality, and consists of health professionals with a mix of complementary skills. These specific areas include, North Wiltshire, Bath & Keynsham, West Wiltshire and East Somerset.

They also coordinate patients’ care with our wider hospice services and liaise with local volunteers to ensure patients and families receive practical and social support.

The delivery of the first year of this exciting development has been supported by several Charitable Trusts and Foundations, who have recognised the importance of our care evolving in this way.

Our deepest thanks go to the Garfield Weston Foundation, the Albert Hunt Trust and the Underwood Trust for their astonishing generosity – we are proud to be working with such respected organisations to help us develop and improve our care.
