Our experienced healthcare assistants support patients and their families, keeping the patient as comfortable as possible within a familiar setting.
Hospice at Home at the end of life
At the very end of life, our Hospice at Home team will work with the family to agree a number of visits to meet the patient’s personal care and emotional support needs.
If a patient requires more intensive medical or nursing help, or is struggling with symptom control, the Hospice at Home team will work alongside our specialist community palliative care teams, district nurses and GPs.
Where available, our team can also offer overnight respite care, sitting with the patient to allow their family members/carers to sleep.
Referrals
We offer Hospice at Home in conjunction with other services. Patients will be referred by one of our Nurse Specialists or another healthcare professional. For example, a member of the Specialist Palliative Care Team and Integrated Discharge Service at the RUH.
Referrals to Hospice at Home can be made 7 days a week by calling our Clinical Coordination Centre on 0345 0130 555
Monday-Friday: 8am-6pm
Saturday-Sunday: 9am-5pm
David and Debbie's story
David’s wife Debbie, was cared for by our Hospice at Home team, providing comfort and support in the familiarity of their home. On the day of their 33rd wedding anniversary, Debbie and David were told she had two incurable tumours on her brain. Debbie was given a prognosis of only weeks.
“Without Hospice at Home we would have been lost as a family. It’s an incredible organisation, full of amazing people. The carers that visited us conducted themselves with such love. They offered practical help and guided and prepared us in a gentle way. Sometimes when they came to visit Debbie was asleep, so they’d sit and talk with me in the garden. Receiving care at home meant we could fill the house with friends and family.”
